Q.When will the tax forms be mailed out?

A. Tax form mailings will begin in mid January and end no later then January 31st. We will make every effort to expedite the delivery of this information to accommodate our shareholders. Please allow 2-3 weeks for arrival. If you do not receive your tax form by February 15th, you may download your tax form online by logging into your account. If you have not registered for online account access, please contact customer service at 1-800-544-5445 to speak to a customer service representative to obtain a temporary pin to register.

Q. I did not receive or misplaced my tax form, how can I re-order those forms?

A. All tax forms are mailed in January, typically beginning in mid January and ending prior to the 31st of January. To allow for mailing time, tax form re-order requests will be available starting February 15th. If you would like to re-order or download your tax forms you may do so online by logging into your account or by contacting shareholder services and speaking with a customer service representative. If you have not registered for online account access, please contact customer service at 1-800-544-5445 to speak to a customer service representative to obtain a temporary pin to register.

Q. When will the Year end statement be mailed and when can I request a duplicate?

A. Year end statement mailings will begin on January 07th and end on January 16th. Year end statement re-order requests will be available on February 15th by contacting shareholder services and speaking with a customer service representative. You may also immediately gain detailed account history online by logging into your account and accessing the account transcript option. This will provide you with the same transactional information provided on your year end statement. If you have not registered for online account access, please contact customer service at 1-800-544-5445 to speak to a customer service representative to obtain a temporary pin to register.

Q. I need the Tax Identification number for Primerica Shareholder Services, what is it and where can I find it?

A. Primerica Shareholder Services-Transfer Agent Tax Identification Number is 58-1713151. This Tax ID# is located on all tax forms created by Primerica Shareholder Services which include the 1099R 1099DIV 1099B 1042S 1099-INT and 5498. Q. My Tax Advisor says I need an Average Cost Basis Statement, but I did not receive one with my tax forms. How do I obtain one?

A. An average cost basis statement is mailed in the same envelope with your 1099-B tax form(s). If you performed a redemption or exchange from a non-money market, non-retirement account, you will receive a 1099B tax form and an average cost basis statement. You may also be able to calculate your own cost basis with IRS Publication 564. You may obtain a copy of this publication by calling the IRS directly at 1-800-829-3676 or log on to www.irs.gov. Please note: There are a small number of accounts that do not receive a cost basis. Those clients will receive a free transcript from 2000 forward, to assist them and/or their tax preparer in determining a cost basis. These transcripts will be mailed the by February 19-23.
